Case Study: Diagnosing a String of Failed Door Operator Inspections at a Brooklyn Shelter
A Brooklyn homeless shelter project had its automatic door operators fail city inspection twice, even after the original contractor replaced components with top-of-the-line brand hardware both times. The real problem was never the hardware — it was a short circuit at the door frame, caused by oversized mounting screws driven straight through electrical wiring hidden behind the frame. We traced it door by door, ordered the correct parts, and had every door fully operational three days later. The client passed inspection within a week, and hired us for every project since — because the previous contractor could not, and did not, figure out why the doors were failing.
When "Top of the Line" Isn't Enough
A general contractor building out a homeless shelter in Brooklyn hired a specialty vendor to install automatic door operators throughout the building — the touch-button, ADA-compliant systems required for accessible egress in a facility like this: press a button, the door swings open, a motion sensor holds it open while someone is still in the doorway, and it closes automatically once they've cleared it.
When the city's inspection team arrived, doors started failing one by one. The contractor's response was to replace the failed components with new units from a well-regarded manufacturer. Inspection was scheduled again. It failed again — different doors this time, but the same pattern: components dead, others intermittent, no clear cause anyone could point to.
Two failed inspections in, the client had already burned real money, real time, and real credibility with the city. That's when they called us — not the original installer, who by that point had run out of theories and run out of the room.
Three Symptoms, One Question Nobody Had Asked
When we got on site, the picture wasn't one failure — it was three different failure signatures scattered across the building:
- On some doors, the electric strike had burned out.
- On others, the motion sensor had burned out.
- On a few doors, the entire door operator module — the whole unit — was dead.
Three different components failing in three different ways is exactly the kind of pattern that makes a contractor start guessing instead of diagnosing. By the time we arrived, the working theory on-site had drifted from "hardware defect" to half-joking talk about the building being haunted. It wasn't the building. It was electrical, and it was consistent, once we stopped looking at each dead part in isolation and started asking what all of them had in common.
Finding the Root Cause: What Was Actually Behind the Frame
We ordered the replacement parts immediately so there'd be no lost time once we had an answer — and while they were in transit, we opened up every affected door, one at a time, to find the actual cause instead of installing more parts on top of an unknown problem.
The answer was the same at every door: whoever had originally mounted the door operators had used six-inch screws to hang them — screws long enough to drive straight through the door frame and into electrical wiring routed behind it. Every one of those screws was a short circuit waiting to happen, and depending on exactly where a given screw landed relative to the wiring behind that specific frame, it burned out a different component: a strike here, a sensor there, a whole operator module somewhere else. It wasn't three separate problems. It was one installation mistake, repeated at every door, showing up as a different symptom each time depending on where the screw happened to bite.
It's a genuinely easy mistake to make if you're not thinking about what's behind the frame you're driving a screw into — use whatever screw is long enough to feel secure, without checking what's routed on the other side of that wood or metal. It's also completely avoidable with the right sequence: check for wiring behind the frame, use a screw sized to the actual mounting hardware, and confirm nothing.
The Fix — and the Clock
The client couldn't afford a third failed inspection. We told them to go ahead and schedule the re-inspection with the city for five days out, before we'd even finished the repair — because by that point we knew exactly what we were dealing with, and we knew it wasn't going to take long to fix correctly.
It took three days. We replaced the compromised wiring and hardware at every affected door, reinstalled each door operator with correctly sized mounting hardware, and tested every door individually: touch-button activation, motion-sensor hold-open while someone is in the doorway, automatic close on clearing, every time, on every door — exactly what the client needed for accessibility compliance and exactly what the city's inspection was checking for in the first place.
The client's inspection passed in under a week from the day they first called us.
Why This Kind of Failure Is So Easy to Miss
The previous contractor wasn't incompetent for replacing parts — replacing a burned-out strike or a dead sensor is the correct first response to a burned-out strike or a dead sensor. The mistake was stopping there. When the same category of problem shows up at multiple doors in slightly different forms, replacing components one at a time treats the symptom and never gets ahead of the failure — which is exactly why the second round of brand-new, top-of-the-line hardware failed the same inspection the first round did.
This is also why we don't treat "which brand is best" as the whole question on a job like this. The client — who had brought us in mid-project specifically because their original contractor couldn't explain what was happening — has since given us every subsequent project on that building from start to finish, and multiple additional projects beyond it. That's not usually how a first job with a new client goes when the reason you're standing in the building at all is that the previous vendor gave up.
